This is a screening tool designed to help identify symptoms you might be experiencing. It does not consider all symptoms or the possible reasons why a person might be experiencing them. This tool is not designed to provide a diagnosis. Only a professional can make a diagnosis. 

This questionnaire is sensitive to whether a person has on-going stress -- they feel continually stressed. Someone who is "stressed" may be tense, unable to relax, easily upset, irritable and jumpy.

Please read each statement and select a description that indicates how much the statement applied to you over the past week. There are no right or wrong answers. Do not spend too much time on any statement.
