Stress Check

This is a screening tool designed to help identify experiences related to stress. It does not consider all possible experiences or the reasons why a person might be experiencing them. This tool is not designed to provide a diagnosis. Only a professional can make a diagnosis.

This questionnaire is sensitive to whether a person has on-going stress -- they feel continually stressed. Someone who is "stressed" may be tense, unable to relax, easily upset, irritable and jumpy. Please read each statement and select a description that indicates how much the statement applied to you over the past week. There are no right or wrong answers. Do not spend too much time on any statement.

We will not share your answers or any information about you with anyone. Please see our privacy policy for more information.
