This scale was designed for people 17 and older, if you are under 17 your results may be less accurate. This screening is not perfect and we advise you to take action if you are distressed and want to seek help even if you score in the low range of severity.

In this questionnaire we ask about a range of experiences, some of which are quite unusual. Please carefully read each statement and decide if your answer is "yes," "no," or “unsure.”
